Please have the following original documents ready when you come for your appointment.
1. Your most current electric and gas bills.
2. Verification of income for the last twelve months for everyone in the household 18 years and over.
2a. You will need a Wage Statement from the unemployment office if: you had no income or a partial income.
3. Please bring Social Security cards or a printout of Social Security numbers and birthdates from the Social Security Administration for everyone in the household.
4. Picture ID
5. Copy of Lease if at least one of your utilities is included in your rent, or you are responsible for paying only one utitily.

No application will be taken without all the above mentioned original documentation.
